  • The Consequences of No Treatment: What could happen if you choose not to proceed?

  • Costs & Practical Details: Any relevant information about the cost, duration, and what to expect during and after.

You are encouraged to ask questions until you feel fully informed.

3. Your Role in the Process

As our patient, you have the right to:

  • Be given information in a language and manner you understand.

  • Ask questions and receive clear answers.

  • Take time to consider your options, discuss them with family, and seek a second opinion if you wish.

  • Agree to all, some, or none of the proposed care.

  • Change your mind and withdraw consent at any time, even after you have given it.

Your consent is specific to the discussed plan. If the plan changes significantly, a new consent discussion will take place.

4. How Consent is Documented

Consent can be expressed in several ways:

  • Verbal Consent: For simple, low-risk interventions (e.g., a routine vaccination).

  • Written Consent: For procedures with greater complexity or risk (e.g., minor surgery). You will be asked to sign a specific consent form.

  • Implied Consent: Your actions may indicate consent, such as rolling up your sleeve for a blood test.

The method used will always be appropriate to the level of risk and complexity involved.

5. Special Circumstances

  • Medical Photography: We may request to take clinical photographs for your medical record to track progress. Separate consent will be sought for any use beyond your personal medical record.

  • Use of Health Information: Your personal and health information is managed according to our Privacy Policy. Your consent for treatment includes consent for our clinical team to use your information to provide that care.

  • Minors & Dependents: For patients under the age of 18 or those unable to consent for themselves, consent will be sought from a parent, guardian, or legally authorized representative.
